Changing the Name on the Subscription Rights

Please inform an NTT East service counter when changing the name on your telephone subscription rights. Applications are received at an NTT East service counter. Applications can also be made by post. Please call 0120-116-000 (toll free) for more information.

Attention

Requests for relocating or suspending telephone service etc. are accepted only from the subscriber. They cannot be carried out if the person making the request is not the subscriber.

Application Procedures

There are three categories for changing the name of the telephone subscriber: transfer of subscription rights, acquisition of subscription rights, or change in the name of the subscriber. Please check which one applies to you and carry out the relevant application procedures.

Transfer of Subscription Rights

Both the current and new subscribers should complete and sign the form 'Request for Approval for Transfer of Telephone Subscription Rights' (Denwa Kanyukentou Joto Shonin Seikyusho) available from NTT East service counters, and submit this along with the necessary documents.

  • Transfer of telephone subscription rights is subject to the approval of NTT East.
  • Transfer of telephone subscription rights is not available for telephone lines under Analog Lite Plan.
  • Please be aware that any unpaid telephone charges become the responsibility of the new subscriber.

Current Subscriber
  • Certificate of Alien Registration*1 etc. with current address
  • Seal/signature

(If the current subscriber cannot visit an NTT East service counter in person, a Certificate of Signature*1,2  is required to verify the current subscriber's signature.)
  • Certificate of Registered Seal Impression*2 (Inkanshomeisho) and seal
  • Employee identification of the applicant
New Subscriber
  • Certificate of Alien Registration*1 etc. with current address
  • Seal/signature
  • Certified Copy of Commercial Registration*2 (Tokiboto(sho)hon)
  • Seal of the company representative
  • Employee identification of the applicant
Handling fee ¥800
  • Price shown is not inclusive of tax.

Acquisition of Subscription Rights

If the name of the subscriber is changed due to inheritance or a corporate merger etc., please submit the form 'Request for Telephone Subscription Rights Transfer/Telephone Subscriber Name Change'(Kanyutou Shokei/Kaisho Todokesho) available from NTT East service counters, along with the necessary documents as soon as possible.

Inheritance Merger
  • Certificate of Alien Registration*1 etc. with current address
  • Official documents to verify the inheritance
  • Seal/signature
  • Certified Copy of Commercial Registration*2 (Tokiboto(sho)hon) to verify the company merger
  • Seal of the new company representative
  • Employee identification of the applicant

Change in the Name of the Subscriber

When there is a change in the name of an individual or company due to marriage, company reorganization, etc., please submit the form 'Request for Telephone Subscription Rights Transfer/Telephone Subscriber Name Change'(Kanyutou Shokei/Kaisho Todokesho) available from NTT East service counters, along with the necessary documents as soon as possible.
